Models of Pharmacist Integration with Primary Care Settings

The 2019–2020 Professional Affairs Committee is charged with addressing various aspects of collaborative partnerships between pharmacists and physicians in primary care settings. The purpose of this collection is for academic pharmacy and other stakeholders to provide information on the various types of models that have pharmacist integration with primary care settings. This information will be used to inform those within and outside of the pharmacy profession.

For this collection, a physician-pharmacist collaborative relationship is broadly defined as any partnership between a pharmacist and physician that provides patient-centered care services, which may include working alongside other health care professionals (e.g., nurses, dietitians). This partnership may occur virtually, telephonically, in-person, or via other mechanisms.

A primary care setting is defined as a healthcare facility that is usually visited first and/or most frequently by patients, which typically does not need a referral (e.g., physician office, ambulatory care clinic, FQHC).
