Collection of Pharmacist Integration Models in Non-Primary Care Practice Settings

AACP Resource

The 2020–2021 Professional Affairs Committee is charged with identifying opportunities and models of integration of pharmacist care services in physician and other health provider practices beyond primary care.  The purpose of this collection is for academic pharmacy to provide information on the various types of pharmacist care services that are delivered by pharmacy faculty in non-primary care settings.

The Administrator(s), Faculty Member(s) and/or Staff Member(s) responsible for external contracts or service agreements with health care entities may be the best person to complete this form.  This person may wish to confer with their faculty member(s) providing the service(s) to non-primary care settings to complete this form or may request that the faculty member(s) providing these services complete this form.  This information will be used to inform those within and outside of the pharmacy profession.

This collection is requesting information from non-primary care settings, which would include community-based practice, long-term care facilities, managed care, specialty clinics (e.g., neurology, oncology), and other settings that are not considered primary care settings.  The committee would like for each contracted non-primary care setting to have its own submission (e.g., separate submission for each contract).

For this collection, a service provided by a pharmacist is very broad and will include many options (e.g., comprehensive medication management (CMM), disease state management, immunizations, medication synchronization) and the opportunity to list additional services.  These services may be provided in many ways including audiovisual, electronic, face-to-face (in-person), telephonic, texting, and via other mechanisms.

Please contact Lynette Bradley-Baker with any questions regarding this collection.
